hey everybody its dr. Bosworth thank you guys for checking in I am here again on Sunday night on our holiday weekend so happy Labor Day to everyone that’s out there I hope you are enjoying the last day of I’ll suppose that summer welcome to Sunday night and this is the night where I live stream from my office in part because it’s quiet and nobody interrupts but also because it’s become a regular schedule of finding a way to answer the questions that I get throughout the week and if I got a great show for you tonight I have a volunteer who’s given me a really good question to use as a teachable moment so ahead of time we’re gonna say thanks to Jerry meanwhile I just want to say thank you to the folks that are checking in hello Angela from Knoxville she’s a trooper a regular I saw Rick Cartwright a minute ago oh there he is a little more volume please okay so I have it turned all the way up and I have another setting on my on my microphone that I can go even higher so if you think it needs to be higher please say that because I will go even higher we are gonna figure out the something if it’s the death of me so yeah you guys are doing exactly I was gonna ask you to do which is tell me where you’re from this is I am coming to you from Sioux Falls South Dakota this is where my clinic is and looking at the different places where folks ask questions from it’s incredibly just motivating and humbling to see how many people are improving their knowledge about the ketogenic diet but also just about their health in general as a physician that’s our first calling is to educate our patients and to really pour into the questions they have so thank you very much for tuning in and I’m going to I’m going to use your your comments as part of what keeps me checking in on a few things like sound and if anything goes wrong I do need to say a special thanks to those of you that were signed up to follow me on Instagram I have reached some kind of magical number apparently 10,000 followers on Instagram turns on a few funky things and I think I reached it like a day ago so I was about 250 people away last week and so I really appreciate you following me on Instagram the reason you might want to do that is that I fast every week and through my fasting I post my dr. Basra’s show you’re gonna learn a little bit more about that tonight through one of the patient stories that I’m going to use as a teachable moment but I really do appreciate those people who also put in a hashtag dr. Boz or actually it’s hashtag dr. buzz ratio so oops I didn’t mean to do that there you go that’s the hashtag if you type that into Instagram I will I try to search that and follow up and give you encouragement ask you question some people ask questions what would you do at this point I’m getting better at checking it a little more regularly so hopefully that makes it easier for you guys to participate as well so I can’t tell you how cool my kids think I am that I have 10,000 followers on Instagram something I said I’m just not going to learn that one and Here I am not only learning it but figuring out now that something magical happened I gotta go take a its guide to Instagram videos or something I don’t know but it’s because of you so thank you so much for doing that and I really hope that you learn by watching what I do on my fasting days how much I use those those ratios and that information to just keep me focused on my fasting and why I do it and then there are so many of my patients here in Sioux Falls and in the Sioux Falls area that also report their numbers if nothing on our at our Friday keto support group but also really like to see that when you when you are checking in on those what what does it mean and why would I do that so we’re going to go through some of that again thank you everybody for telling me where you’re from and I haven’t had anybody else say anything about the sound so if you think it sounds good I’m gonna flip the switch and we’re gonna start our little lesson for tonight and use the this man named Jerry has given us a really great teachable moment you won’t see the comments here for while I do the while I do the well I do this little lesson but when I come back I totally would like you to post some of the questions you’re asking or even repeat some of the ones that you’ve just put in here I will use all of that information as I as I go through oh thank you for Ken for saying that sound is good Oh everybody else that sound is good okay that’s what I needed to hear before I could flip the switch because I cannot see your comments when I do this next part alright so again we are going to hop over to our our little keynote our a little lecture it’s actually well I’ll tell you one other thing before I do this I have a couple of like celebrations that I have been asked to do one of them is that the obstetrics and gynecology has their national meeting coming up in New Orleans and I’ve been asked to be one of their speakers so a lot of the slides that I’ve been working on and some of the just making it how can you put the the most commonly asked questions or some of those things that I think your OB gaion would want to know about a ketogenic diet and what their patients would be asking but I have 30 minutes to put it in there so that means my slides need to be really well done that the animation and the information needs to be a way they can hear and read some pretty intense information in a short period of time so some of the stuff you’re gonna see tonight is in a little bit of prep work for that but mostly it’s to answer Jerry’s questions so we’re gonna hop over it we’re gonna show you what Jerry’s question was and ahead of time I’m just gonna say thank you so much Jerry for being the being the the guy who wrote in and said here’s my praise and here’s my question so let’s begin all right so this is a question this is actually the email parts of the email I just took a few excerpts out of this email that that Jerry wrote in dear dr. Baz we are on a cross-country trip in our motorhome I want to thank you and let you know the profound affect you and your videos have had on me and my wife Michele last November I had diabetes for 20-plus years I had a fasting glucose that was out of control a hemoglobin a1c of 10.1 I weighed 255 pounds at the tall height of 5 foot 8 inches this calculates out to a body mass index that is in the extremely obese anything anytime that BMI is above 35 the doctors will file that in extremely obese so really struggling with his weight at short-acting insulin Jerry paused at this moment and said I told him I needed to think about it and get back to him that was in November in December he started looking remembers seeing some videos on YouTube talking about curing diabetes and he found my channel while I found you while researching after much studying I decided I could do it and I told Michelle she immediately agreed and we dove in oh let’s see here okay so I just got a text from somebody where did she go I thought it was me oh she had to laugh okay so let’s see I’m gonna see if anybody else can see me right now let’s do this one here okay so I am going to sit patiently because this has got a two or three minute delay and I want to see anybody tell me that you can see this so oh it’s back okay so I’m gonna try doing the keynote a little bit differently we’ll see if this works okay good here so I wonder what you guys heard I’m what was the last slide you heard what okay sit tight because you aren’t gonna be able to understand this if I if I skip too many of the slides so I will wait one more minute here I’ll keep going he says okay all right I’ll keep going so I’m gonna try and push play on the on the actual slide deck again and we’ll see if that bites me in the butt so if it does I’m sorry I did try to make it as beautiful and lovely as possible all right here we go we drove in okay good all right Ludo VIN so here he was finding those videos doing some research we dove in awesome okay I wish I could edit all that out but here we go we ordered your inspirational book any way you can and dr. Fong’s both of his books as well as dr. ken Berry’s books we followed the advice in the books and lost weight next in eight months over that period of time in eight months I have lost 70 pounds he now weighs a 185 pounds which has brought his body mass index out of the extremely obese and he’s actually graduated out of the obese into the overweight section a BMI of 28.1 is a only about 15 pounds away from normal wait so incredible accomplishment there dropped a shirt size from to XXL to medium and his pant size went from 32 inch weight or yet 42 inch waist to a 34 inch waist his mental clarity improved he vastly improved his mood and we’ve talked about that a couple of times on several of my channels it is the reason I push folks into trying the ketogenic diet is watch what your brain will start to do he had the disappearance of the tingling in his hands and feet otherwise called neuropathy that is really important in measuring risk for dangerous parts of having diabetes so the neuropathy all but disappeared he says there’s a little bit left but most of it’s disappeared and the inflammation that was in his gums according to his dentist was all gone and he wasn’t as aware of it but his dentist was remarked how much his gums had improved and his eyeglass prescription also improved finally the ophthalmologist who had been giving him steroid shots into the I said hey guess what we can stop giving you these shots into the eye because your inflammation has improved so much all right so here comes the kicker I saw my doctor in June and he thought all my improvements were nice he said a couple other sentences where you could just hear the disappointment that I’d done all this I was so happy with all the things that improved but the doctor kept returning his focus back to my blood sugars and my a1c he wants me to go back on insulin even though I told him I think the insulin caused all my problems he’s the patient stopped taking insulin in February when his fasting blood sugar was down in the 80s so his question is my blood sugar is very stable at about 225 his hemoglobin a1c is down to 9.1 his ketones run about 2.0 and his weight is stable at about a hundred and eighty-five which is that body mass of twenty eight the in the past few weeks I know I could stand to lose about 25 or 30 more pounds my goal is up to be a hundred and fifty five which would be an awesome goal that would put his weight at 23.5 again the reason I keep pointing out these BMI is because it does predict that diabetes and the insulin needs I feel better now at 68 that’s the part of this but I think is so amazing he did all this at 68 years old I feel better now at 68 than I have in twenty years with all the markedly improved markedly improved results I am struggling with what to do next after he met with his doctor he said obviously I’m concerned about my blood sugar and my a1c and here are his questions that we’re gonna try to answer tonight he wants to know is my pancreas truly burnt out is it not gonna be able to produce the amount of insulin that I need can twenty years of diabetes be cured and that’s what this is this lecture is about tonight why are my glucose levels so high is glucose hiding in my body do I just need more time to improve the readings and then he says I do intend to work on fasting and exercising more but do you have any suggestions that I can use or thoughts on what is happening to me best regards Jerry so first of all Jerry thanks for sharing your story it is these teachable moments where you’re gonna see me come alive I I loved teaching about medicine but there is a difference between mythically teaching someone and actually saying if you were my patient let me show you some of the things I’d make sure you understand so we’re gonna start out by going away away from the story and giving a few back stories so you can see why I advise what I’m about to advise to Jerry number one this is the bone marrow and that red section in the center is where three major types of cells come from one of them is your red blood cells that’s what carries oxygen around to your body one of them is your white blood cells and that is what repairs your body but also defends your body against infection and finally your platelets which are parts of how your body doesn’t bleed to death but also how your body doesn’t have clots where it’s not supposed to so again red blood cells white blood cells and platelets all those other little pictures are interesting if you’re mean but probably not to you so I want you to think about the bone marrow and those those cells are coming out of the bone marrow every day in Gerry we’re gonna focus specifically on these two types of cells those little red circles that are kind of dome shaped in the center are your red blood cells and those pink and white ones represent your white blood cells you will again have usually many more red blood cells on my blood cells and we’re gonna talk about what Gerry showed me in his in his lab report so this is a picture of a red blood cell and those little dots throughout there are hemoglobin molecules you have millions of hemoglobin molecules in each of your red blood cells and they are there to bind up oxygen and deliver it to your body to get it all the way to the tips of your fingers inside the tips of in your organs into your brain and those hemoglobin molecules are super important for the health of the person when we look at oxygen molecules coming into our lungs our body will put them inside the red blood cells and then that red blood cell delivers these oxygen molecules all throughout the body we’re gonna use the muscle as an example here but I don’t think it’s an accident that in recent months you’ve seen Jerry’s tingling fingers and tingling toes do a better job we’re gonna kind of show you why when I look closer again this is red blood cell and we took the hemoglobin again there’s millions of hemoglobin particles in every red blood cell but we blew it up so you could have a little better understanding why you’ll see me make sure that when people are looking at improving their health there are four iron molecules in each healthy hemoglobin and as long as that iron molecule is where it’s supposed to be each one of those hemoglobin proteins will be able to carry four oxygen molecules again millions of those in each red blood cell and you have lots and lots of red blood cells as long as you’re not iron deficient your body will be able to deliver those oxygen where it’s supposed to and this is where Jerry’s blood sugar is gonna come into play so here’s a bunch of those little squares and in my cartoons I like to use these little squares as your glucose and you’ll see some of them have happy faces some of them are kind of laying down some of them have sluggish eyes because in our body glucose is super important to get the job done but as the glucose inside the blood vessel inside the blood circulation gets higher and higher and higher it does some strange things to our system and we’re gonna talk specifically about what it’s doing to Jerry’s system and how we’re gonna help him get to the next level of his health when I look at that blood stream I want you to imagine every single one of those glucose little squares those little rhomboid kind of characters they are surrounded by swelling or inflammation they attract water so each one of those little glucose molecules floats around the system with a whole bunch of Boggy ‘no surround them it’s kind of like a slimy moisture that goes with them and when the glucose is high the inflammation around your system is high when we look specifically at these red blood cells as they’re floating through your system the red blood cells get glucose stuck to them and in in actuality it’s being stuck to that hemoglobin molecule I showed you in a couple of slides ago within that kind of protein that the sugar gets stuck and it gets very sticky so that’s the job of these red blood cells and I left one white blood cell on there just to kind of make sure you’re not forgetting there’s other cells that are affected by this but when we look at those red blood cells and the job they have to go to the tips of Jerry’s toes deliver that oxygen and be able to make sure his fingers don’t get low on oxygen and start shutting off which is why they will tingle they’re not getting nourished well enough when he has those really high blood sugars and part of the reason why is because his hemoglobin is full of sugar it’s full of stickiness when you look at these red blood cells in front of you these four red blood cells they exit out of the bone marrow and they’re gonna last for about a hundred days so about three months so if you’ve ever seen us talk about a hemoglobin a1c or this 3-month diabetes report card what we’re really doing is getting inside your red blood cells opening up to find those millions of hemoglobin molecules and we’re looking to see how much glucose is hanging out in your hemoglobin molecule we know that if it does if it’s carrying a lot of sugar with it you are not gonna see your bread blood cells do a very good job of supplying oxygen to your heart to your brain to your tingling fingers you’re gonna see parts of your cells start to die not because so much of the sugar but because of what the sugar did to the hemoglobin it can’t deliver oxygen and these red blood cells have a hundred days to float around your body and gather sugar so when when we look at the next slide here you’ll notice there’s those glucoses floating through your system and we all have glucose in our system as our rug blood cells live in our in our arteries they will be exposed to a certain number of of glucose molecules and that will allow them to be sticky or not sticky so if we look at a hemoglobin a1c of 4.0 that’s actually what I like to see my patients glucose or hemoglobin a1c has come back at is if they can get between four and five I think they are in golden heaven not only do I know that the inflammation on the insides of their body is lower but some of those complications that Jerry is struggling with like his I had so much inflammation that the doctor was putting a needle in his eyeball to help him to get that to inflammation to be lower and when the eye doctor said it got better I know for certain that that’s not retinopathy that’s not the part that says you know what your eyes tried so hard to fight this high inflammation that it made some extra blood vessels or it killed off some of the other parts of the eye because that that red blood cell that was supposed to nourish the eye it didn’t do a very good job because it was filled with sugar so when I look at Jerry and I say what would be my goal for Jerry’s hemoglobin a1c I would want it between four and five let’s just keep showing you a little bit more about this if your hemoglobin a1c comes back at five it means the average glucose and that means during eating after eating before eating when you’re sleeping when you’re awake when you have an infection the average hundred days of those red blood cells floating around your system has a sugar of about a hundred when we are measuring our finger stick blood sugars you get a hemoglobin a1c of six it’s one thirty six seven up into the 170s now you get into Jerry’s range where he has hemoglobin a1c of 271 or nine it wasn’t quite nine maybe nine point one of almost 240 when your hemoglobin a1c gets above 12 you’re in the 350 range in the when it’s above 13 it’s almost up to 400 blood sugars so when what doctors are doing when we see these hemoglobin a1cs yes we are extrapolating to say what is your sugar and you can tell us that by looking at your finger but more importantly we are understanding how well your body is able to repair by delivering a red blood cell to the to the tissue and asking in our minds how well nourished is that a part of the body because of how well his red blood cells are working at that stage of of the treatment so as we look at some of the things that when Jerry says 20 years of diabetes I think how sticky are his hemoglobins and how well can they deliver nourishment to his brain in addition I want to know what’s the chances of him having a stroke what’s the chances of him not being able to give all that oxygen that his brain requires at the time the brain requires it and it has everything to do with how well do those little red blood cells work in Jerry’s circulation we don’t we can’t get away from diabetes without talking about the risk of heart disease again it’s not just the blood sugars that we’re thinking about every single one of those glucose is floating down the system is impacting those red blood cells they live for a hundred days and it’s their job to give oxygen to the heart to repair any low oxygen tissues kind of like the tingling fingers and tingling toes as those cells try to hang on saying we’re trying to give Jerry every message possible to give us better oxygen and that means he’s gonna have to help us get that sugar down which he did and those nerves woke up again and don’t hurt him or send signals when they’re not supposed to in many ways the tingling fingers and toes is one of the bigger signals to me that says every day those cells are trying to die and you have the little spirits of the cells saying I’m gonna give it another day I’m gonna try to make it another day maybe tomorrow the inflammation will be lower and Jerry was able to hang on long enough that he reversed that or at least mostly reversed that and at 68 years old that is such a blessing now as we look at what Jerry’s doctor is faced with he walks in and says doc I used to have a hemoglobin a1c of 10.1 and now I have a hemoglobin a1c of 9.1 and when I was reading all these things on the internet I stopped my insulin because I had some fasting sugars that were in the 80s and if that was his sugar every day I would have said by golly that is a good thing cut down that insulin you don’t need a lot of insulin if your morning fasting sugars are 80 but with a hemoglobin a1c in the last you know thirty to sixty days we’re gonna know that his sugars are at least above a hundred or at least above 200 again or still I I don’t doubt that he has morning fasting sugars that look pretty good but what I’m worried about is what’s happening when he starts stirring up inflammation or when he starts asking his body to do what it’s supposed to do but it’s got this history of needing lots of insulin and and having lots of high blood sugars and by giving Jerry that long-acting insulin his sugars will come down and then magical things start to happen just wait we’re not quite done yet hang in there I want you to notice also that I when when we took away I took away some of those sugars to show you what happens when you did decrease it from 282 to 246 well by golly there are less stickiness so the less glucose is found in each of his hemoglobin molecules and his red blood cells are doing a better job at nourishing his brain he said his mood is vastly improved his concentration was better he is inflammation around the gums in his mouth again a place that buds know if we want to find the easiest door into the human body boy that tissue right next to the teeth is a very easy sneaky way to get into our system if your immune system isn’t working right it will win the bugs will win they’ll get in and your dentist will be the first one to see that so as I look at what we hope to have is glucoses red blood cells that don’t have any stickiness or very little stickiness it found in their hemoglobin and we’re gonna get to what we would do with Jerry in just a minute I don’t want to miss this other teaching point though and that is what happens to our white blood cells so those red blood cells they last a hundred days so they give me a very long time to say I know Jerry’s done a much better job in recent months because he lost 70 pounds he is down to almost a normal body weight and he did this at 68 years old I take care of chronic disease I am in this for the long game and to get and somebody over the age of 65 to lose that kind of weight it is a profound improvement in Gerry he needs to take this as a total win but his body has some limitations that are gonna lead to some stumbles if we don’t help him it’s this moment where I want to point out what happens to your white blood cells so as white blood cells enter your circulation they only last from a few hours to a few days so you can see a whole new crop of cells enter into your circulation and then their job is to fight off those infections by the teeth repair the body when it’s been injured if you have a stroke or you had a little bit of low oxygen in your fingertips and you want the body to repair it’s these guys that need to do their job and improve that and when you have really high blood sugars this is what happens to them they’re what the white blood cells also get a stickiness to them they get kind of gummed up and when they try to navigate to their target to go in and fight the infection they get stuck think of it as sticky they’re kind of like glue like and it’s not exactly like that but think of it that way that and those sugars are high I visited a very dear friend and patient in the hospital this past week and he was having a major infection in his replaced hip we had to have the hip socket emptied take out the hardware and let him heal because there was just pus infection in his hip during all this he got a he got attacked by his own white blood cells they they they failed on him they had a coup they went in and started attacking his joints and then we were all very concerned because the reason he’s lying there in the hospital bed is he had these awful infections so instantly we said what’s the best thing we can do for his white blood cells and we checked his sugar well in that hospital bed and it was 222 and the family was kind of concerned because he didn’t have much of an appetite and my advice was don’t let him eat if he’s not hungry please don’t feed him and when you do feed him give him something like bone broth or just no sweets no sugar give him high fat meals so his satiety is low and you’ve got all these medications including steroids that were giving him to help that risk that’s going haywire and just to help him through this very difficult time he’s in his 80s his white blood cells are completely failing because the sugar was so high just a few meals where he skipped the meals and the sugar went down and his body started to rally in part because of all the things they’re doing to that patient in the ICU but much like Jerry it’s something you don’t think about that these little white blood cells are trying to be your defense system and when they’re sticky and gummy they’re not going to work very well as you lower your blood sugar these cells only last 48 hours so you get a new crop of those every few days and especially if you’re fighting off infection your system will really send them out of the lymph system and say attack helped us survive and you can see quite an improvement in your immune system in much shorter order as opposed to what happens to your white blood cell or to your red blood cells all right let’s go back to Jerry so Jerry again said his my pancreas truly burnt out can twenty years of diabetes be cured my answer to Jerry is maybe your pancreas is is not meeting the needs of what your Sugar’s are right now and we can confidently say that because these blood cells that are floating around in your circulation for three months they are super sticky still we need those sugars to be lower and you recognize that in your report you say I see there’s something wrong with my blood sugars how do i what do I do now do I go back on insulin is that the right answer and I went through this whole story to say what what is the doctor learning and why was he so concerned about your blood sugar and your hemoglobin a1c and it is in part because he knows if your red blood cells are that sticky you’re going to have first of all your metabolism is gonna stabilize out you’ve had this really great success of losing 70 pounds in eight months at sixty eight years old that’s amazing we want you to keep going we want you to get to the part where you don’t need the doctor but right now your system is saying I can’t do much more my good my hemoglobin is too sticky I can’t I can’t get to the next level when your pancreas is producing insulin it’s clearly not producing enough to match what your system needs does that mean it’s burnt out or does that just mean that it’s producing buckets and buckets of insulin and the insulin is resistant it’s not working as well as it should which leads me to the other questions you wrote in why are my glucose levels so high is it is the glucose kind of hiding in my body and in part yes you’ve got a ton of storage that you’ve put into your system you can call it a fatty liver and it’s it’s it’s not from eating fat it’s from eating excessive sugar or excessive carbohydrates for the better part of 20 years and your body says I cannot find another place for the sugar we can’t keep up and it’s floating down his red blood or his veins and arteries we got to find another place for it and it will stuff it into the cells of your liver in fact your liver will keep making more cells just to use as storage for keeping that sugar in a better form or a more concentrated form which is the fat the triglycerides in your in your liver as well as the glycogen which is stored sugar in your liver so yes you have glucose hiding out in your body and do I just need more time to improve time is going to help a lot of this but like many of my patients when they run into this what I would say a plateau you have matically improved your metabolism your system went from a nightmare like you’re gonna die there’s there’s a lot of problems with a 20 years of high blood sugars and ten point one hemoglobin a1c 68 years old is young you can’t you can’t do that yet you got it you got the best years ahead of you and this is when wisdom shows up so in order to be granted those we need that body to do what it does which is repair and that means those sugars need to be lower in whatever way we can do that the insulin whether or not that’s the case is gonna come down to the next couple of teachable moments I need Jerry to start doing dr. Basra’s show now at 68 I will totally honor him if he figures out Instagram because I’m struggling with Instagram but if he posts his sugars you’re gonna see that my fasting starts on Sunday and I will fast until I get a dr. Baz ratio of 40 and what I want Jerry to do is more carefully be checking his his doctor Baz ratio as he does that he’s gonna take his glucoses and he checks his glucose and he’s gonna take his ketones and divide the little number into the big number so the glucose is the big number and then you divide the little number into it and what we’ve learned is the ratio will show us that as that number gets if we could get his doctor boz ratio consistently under at least 120 you would see the need for his insulin to drop you would see his insulin resistance improve and just guessing where he’s at with a 2.0 is about where his glucoses have been but he’s had sugars that are at the 200 to 225 range I would want him checking that dr. Basra’s show two or three times a day for the next couple of weeks now that’s a bit of a bit of a bite that’s all I mean as a diabetic you’d like them checking their Sugar’s a couple times a day but most of my type 2 diabetics I have them checked first thing in the morning and and I didn’t have him check it any other time but when I’m trying to make a metabolic shift when I’m trying to say can we see his body heal or they’re stuck at this plateau where you see your sugars doing one thing your ketones are doing something but it’s the combination of those two that helped me see that helps your doctor see what is Jerry’s insulin how much insulin is his body producing and does it match the needs of his body instead of like hopping back on the long-acting insulin I would have him check this several times a day for a couple of weeks and take them in to his doctor his doctor will know what to do and if not they know how to get a hold of me Instagram apparently but again and we can’t give him medical advice I’m saying if I was his doctor this is what I would start with is get me some numbers I want to know your insulin level and I don’t want it where you’re sitting in the lab you go into the lab to get your insulin checked and although that’s a great thing to check it’s not something I have most of my patients spend the money on I have them spend the money on a glucose meter and you can see in the show notes below a for a care with if you type in Dr Biasi you get a discount on the for care six which is a tool that lets you see your own healthcare and that really is what I’m trying to ask ask Jerry to do that if he can get his doctor bas ratio under 100 that’s goal number one but the next thing he should be looking at is at least once a week I would want him to fast to get his doctor boz ratio 240 or less and I’m telling you that could be a long time for Jerry he might have to fast for three to five days so that isn’t safe with his doctor isn’t on board and watching him and helping him the medications that I would have a 68 year old diabetic with a hemoglobin a1c of nine or ten I can list him in my head what I would have him on his blood pressure would probably be harder to control maybe he’s on metformin maybe he’s on some other drugs that stimulate insulin production all of those need to be assessed by his doctor because if you were really saying could he possibly resurrect his body into a healthy non-diabetic improved give him the last 10 years 20 years whatever God gives him of his life the key thing is is he repairing the body faster than he’s aging and that means we’d want to touch Auto fiji at least once a week the rest of the time I’d want his doctor baz ratio less than 100 and he’s gonna I mean I can tell you what he’s gonna ask he’s like I got a sugar at 220 how am I supposed to do that and that’s where this comes down to what is it that you are doing this for so I point out that one of my cancer patients show up and especially if you’ve read the book that I wrote I’m going to come back to the last two slides in just a second but give a different screen for just a minute that dr. Basra show isn’t isn’t easy and I get a lot of questions let’s say well what do you do after you hit 40 and I on purpose try not to answer that because I have so many patients that look to me and follow exactly what I do that I don’t want to discourage them because I I fast – dr. Vaughn’s ratio of 40 once I hit it I’m not perfect I do some things that are naughty I celebrate with food sometimes I’ve tried to do that less but I usually am a one meal a day so I try to make it to the afternoon meal which usually is a 2 or 3 o’clock in the afternoon but I eat till I’m full on that meal and I probably eat more than I should but I like it and I kind of feel like oh I deserve it because I made it to 40 and I don’t often check my doctor Baz ratio after that but it’s also because yep I could lose another 15 pounds it would be a perfect world if that’s what I would do but when I hit the doctor Baz ratio of 40 my personal goal is I’m working on not celebrating I’m working on just going back to a normal when I looked at what I had my mom do and in this book if you haven’t read this it truly is you know one of the biggest ways we supported is the people who have written a review for this book or have bought this book bottom for their neighbor bought it for their preacher bought it for the doctor it truly is a the teaching tools I wrote for my mom when she was fighting cancer and when the cancer was about to take her life we were searching for a doctor buzz ratio of 20 or less and people said all the time I’m like yeah all the time and you get to the part of that the book where we had kind of fumbled our way into surviving it where it’s probably 2/3 away through the book and then a left turn and hits her and we have to do some extreme things and I tried to do it with her but I failed but she was facing her life-or-death situation in many ways like what I think Jerry is facing is he has this amazing gift that says you lost the weight you can see parts of his body trying to repair trying to improve the the the inflammation that has happened to his system over the course of the last you know two decades and now he wants to know could I care my diabetes and I’m like why sure you could you would just eat very very few carbs maybe zero I pushed when people are really looking for the fastest repair my my diabetics that are in extreme situations like facing a amputation or my folks who are trying to treat seizures with this or my folks that are trying to fight cancer with adding a ketogenic diet I would have you google Paleolithic ketogenic diet which means there’s no dairy and I’ve tried this from time to time to because it is incredible how much it boosts your your ketones and your metabolism decreases inflammation but it is not easy and I don’t want you thinking that I do that because it’s it’s too much for me at the stage that I’m at and I don’t have the urgency that that maybe Jerry does so when people say but do you do a dr. Baz ratio of 40 forever I’m like no I try to hit it once a week how do I hit it I fast I means black coffee salt and water that’s it and it usually takes me 36 hours plus to get there sometimes I get there sooner but I really I kind of set hitting 36 hours of fasting and a dr. Baz ratio of 40 and if I’ve been good the week before sometimes I can hit it in 24 hours but there is an amazing difference in what’s happening in my system versus what’s happening in Jiri system so when Jerry says could I reverse diabetes I’m like yes you are a type 2 diabetic it is clear that you make insulin you just aren’t making enough for the amount of sugar that’s trying to either come out of the organs that have been stuffed full of sugar for the last 20 years or that your insulin just is still very resistant so the way you fix that is you shut off the stimulation of insulin and that means you fast you are 68 years old with several other medical issues though so please include your doctor when you do this but when I look at what I did with my mom is I used the doctor Baz ratio to help her reach a ratio of 20 or less and she did that for a very long time so let me just finish these questions out and then I’m gonna come back and we’re gonna take a few questions so hang tight oh we’re gonna just finish this up all right so Opitz gonna play that slide for you again again here we were looking at the doctor Paz ratio if you can get it under 80 we have a pretty good chance of otology and weight loss but the last thing was is one of the suggestions that I would do for Jerry and what do I think is happening so that’s in part what I was trying to answer with that again Jerry you are looking at a huge improvement that you’ve made since in the last eight months and now I’m going to ask you to do a little bit more where I want those ketones hi enough as we figure out how to get your glucose low enough and in part those are learned skills of decreasing the glucose spending time where there’s no signal for insulin coming into your system and you’re not gonna be perfect you’re 68 years old you’re gonna have chapters where you just can’t do it all and I am here to tell you there are options out there and I have some of these in the show notes to say use the ketones to help you when you when you’re having times of weakness when I am struggling I will use ketones in a can which is what I call them and ketones in a capsule those are BHB and although they’re not a hundred dollars a can anymore which is what they were about five years ago they are expensive I use them whenever I having a time of weakness and I know that it’s me being a human being and I’ve made a choice that’s not perfect and I’m trying to say okay I want to undo that and I don’t like the inflammation and how long it takes my body to undo that when I when I have had too many carbs or I’ve had a glass of wine remember that if you do use MCT oil this is the very specific kind of fat that is not digested it is absorbed and this actually has out on the market as of this week where I put BHB and the the good MCTC 8010 not the not the c12 and not the c6 into one product and that allows you to have a quick response of the ketones in two to three hours but instead of having to sip that for the next three or four you know like you can sip it for four or five hours if you’re just having BHB when you add c8 c10 into that same drink you now get the production of ketones coming out of those mitochondria in your liver for up to six hour five to six hours so you get a quick response with the salts and then you get the longer acting ones with the these very special fats called MC TCH c10 and those are the things that I would say have them on hand even if you’re like I don’t want to do this every I don’t want you to do it every day I want you to do it when you’ve screwed up because if we’re gonna heal Jerry’s 68 year old body we need to have him be a human and when he’s human and screws up get him back into ketosis as quickly as possible his body will get stronger each week of course the best way to do this is to fast but I don’t know if Jerry’s other health problems will allow him to fast for five days to get his ratio to where it needs to be I do know that if he checks that dr. Baz ratio and he posts it I hope to connect with him and at least give him encouragement but I would have you do that too I am trying to make this something on Instagram that we follow and if you’re looking for encouragement or you have questions about what you would do about your doctor Maas ratio put in that hashtag and send me a message and those are the kinds of things that when when I look at you know how many people are now watching this this channel but are also improving their health like Jerry he did something really important he took ownership of his problem his doctor said more insulin and there’s some little ping inside his brain that said all right I really really need to make a change and when he does let’s let the whole world catch him and say let me help you because it’s gonna be good for a while and then it’s gonna be hard again and then it’s gonna be good for a while and then it’s gonna be hard again I’m gonna take on mr. s Olson or maybe it’s a Miss I don’t know I don’t understand how exhaustion is Ketones help so let’s take Jerry’s chemistry for a second his body is used to burning sugar he has glucose everywhere he has blood sugars in the 200s that’s on average and I know that if a ketone is in his circulation it goes through the cell membrane and it gets in front of the glucose to help to burn as a fuel and you’re like wait a minute I thought you have to burn glucose before you can make ketones like yes if you’re fasting you do your body doesn’t make or use fat until those glucose are burned up so Jerry’s stuck he has really high sugars he’s gonna have to like I don’t know be staplegun to the bed to stay away from food long enough and his health isn’t healthy enough to say just fast your way through that he’s not healthy enough but if his mitochondria keep practicing using ketones by adding them to the circulation his key his mitochondria a gets stronger it suppresses his appetite oh by the way it wakes up part of his brain his brain now will use ketones before they will use glucose so that’s amazing improved research we’ve seen in the last two years one of my favorite parts of the metabolic health summit is the updates on the neuroscience about how we can see brains preferring the use of ketones in problems with dementia problems with unstable mood difficulty focusing you know from ADHD to depression we know that when the ketone is available the brain will start to haul it in across the blood-brain barrier and use it and you can hear that when you look at Jerry’s when you look at Jerry’s email but he wrote in he’s like I’m I have immensely better mood my my mental clarity is like I it’s like an awakening and I’ll tell you that happens in my clinic and it’s not that Jerry is going to need to have supplements forever but his health has been stressed for so long that I want his mitochondria burning ketones most hours of the day and his body can’t make them as consistently because he has really high blood sugars so by adding the ketones in circulation and keeping them around for at least eight hours of the day he’s going to find a strengthened metabolism every week instead of the plateau that he’s at like I have my diabetics that come to group in Sioux Falls when they do their fasting I tell them if you get to a part where you’re hungry drink ketones add ketones so that you practice using those and they make it you know 48 hours they do step over the threshold and their bodies get better at using ketones and then you watch what happens over the next six to eight weeks they need it less because those blood sugars are coming down I don’t like Jerry’s blood Sugar’s being that high it’s really hard on his red blood cells they’re gummy his his white blood cells are not gonna be as efficient at fighting infection and he’s going to pay a higher price if somebody doesn’t teach him that like we gotta get those sugars down so if you don’t want to go on insulin we we got to lift the ketones and we got to push the sugars down which means maybe that’s a meal every 48 hours maybe in between those 48 hours I don’t expect you’re gonna wake up tomorrow being great at fasting there is it is not an accident that civilization has linked prayer and fasting together for culture after culture and religion after religion that’s real it’s hard you’re going to need to strengthen this mental soul and you know Jerry’s just trying to get out of the diabetic range and that’s that’s a lot to ask of him so if we can help him by saying what it is 2019 guys this is amazing we can help you it doesn’t need to be forever but it sure is impressive how much I’ve seen when they say just keep adding the ketones get that dr. Baz ratio to 120 or less and keep using either enough ketones and fasting so that you won’t need them forever but okay do you think carb cycling after losing all the weight I need to and okay hold on I gotta go ahh I needed to and being heat Oh for a couple years is a healthy thing ok so just me says do you think carb cycling after losing all the weight so again that’s a different problem than what Jerry’s fighting with but I get that a lot is the carb cycling one it caught my eye where I’m like ok and that comes up a lot the carb cycling there is a whole video I did on this so that you see if you’re trying to strengthen muscle mass as an athlete I would not recommend carb cycling there’s a difference in you want that key ketosis setting it doesn’t mean that they can’t use more of the of the sugars they are better at they are better at burning glucose athletes are but the carb cycling in an athlete if you want the best strength and the best like growth hormone production you better better be checked you got to have a good doctor bas ratio in order to to accomplish that all right a couple more things that I can’t also see how many people are watching but I do want I’m gonna ask you a couple of other favors here so I want you to like this video again I keep learning things it turns out that even if you never leave a comment if you liked the video I thought that was just a Facebook thing I’ve never told my face my my my youtubers to like it but apparently this is how other people see it it’s kind of like asking people to write a review of the book that’s not in my want to find the one who is eating potato at so ok so I think that’s the same guy who wrote in just a minute oh just me says I tried eating some potato salad and peaches and this summer and I have to say it left me feeling more like I used to I think that is the power of what do I see with the supplements again I did not start out when I was writing this book the data was not even nearly as rich as it is today as I look at the lectures I’m preparing for the opie guy and that I will speak to and I think I just been invited to speak I think in Puerto Rico so I’ve never been to there so I thought I’d say yes to that one but what I it’s a Quito conference that you say what can I bring that the new learner literature has talked about in boy I’ve fallen off the wagon and I’ve lead my support group within my community and everybody at that table has fallen off with potato salad beaches and it sounds like well of course we would do that and then when you’re the one who’s had that icky feeling of my brain works slower my energy is sluggish I my gut rumbles because it feels better than on the other side and to get back over to the other side faster that’s how I was when I use a supplement and it’s much less maybe once so every two week once a week once every other week and it really is just a faster way to say I don’t like feeling like that I don’t like living with the regret of the decisions that I thought would be okay because oh I’m healthy but when my patients get stuck there and their brain starts to do things like it used to and it slows down sometimes they fall off and I don’t come back to group and I I really find that you can shut off those cravings by adding the circulation of ketones and you can improve your inflammation so that you can get back to doing what you know you should do all right it’s the last time I also put out a call for anybody who wants to start their own keto group again I look at what I can offer to the keto support group and I try to use the talents God has gifted me with I love people I love teaching my classroom has grown to an incredible size on YouTube and now on Instagram but I also know that there isn’t a there isn’t a better way to learn that having a group of people to sit around and be accountable to and it doesn’t need to cost you something you need to have accountability so I don’t care if you call it you know keto support group or if you call it a book study or if you call it stitching bitch I don’t care whatever it is that you get together and you are in a group of people that are trying to improve your life I would encourage you to use the playlist that we use in my support group it is the original playlist that’s called beginners and it says all things keto that playlist is short it’s got the basics for the keto diet and then you know these videos are meant to be more advanced lessons but also to show you when when you’re looking at the science it really is here if this is not a fad diet I will be on this till the day I die I will use this in my medical clinic until I no longer practice and I am looking to expand my practice outside the state of South Dakota so as I do that I would love for you guys to reach out to me after your support group has met three or four times I would zoom in and answer some questions for you as a way to show my support that support groups are how we help this country have a better health profile it’s how we help people like Gerry he needs accountability he needs somebody to show up to every week and when he stopped showing up somebody needs to pick up the phone and say where you been we use a secret Facebook group in my support group but nobody else can see but it really is encouraging to watch that what the patients are doing and it’s it’s amazing after you add you several people on your in your support group how much they help one another so I am gonna use that as a no it says 587 people watching now that’s amazing okay well
